I LOVE Nob Hill Cafe, right next to Grace Cathedral. Small, intimate, great pasta. Come early, lots of people like this place. over 7 years ago

Nob Hill Cafe mentioned above is great. North Beach is very touristy, go for a nice coffee and pastry or breakfast in that hood instead (Cafe Trieste to get a great vibe of locals and all their color!)

I enjoy Italian in other neighborhoods. Just on the fringe of the main North Beach strip is Cotogno (more in the Gold Coast neighborhood, just a block away from Broadway). Superb food and great space, with large windows to watch the foot traffic.
Perbacco and Barbacco on California Street downtown are also fab. They cure their own meats, have a superb wine list, and are great for either lunch or dinner. Barbacco is right next door, same owners but lighter menu and slightly easier on the pocket. Both are great for a twosome, or larger group. over 7 years ago

Oh and yes, Pazzia in SOMA is also terrific. Small spot that serves up great pizza and daily specials. Great spot for lunch. over 7 years ago
