Hello Tony, you have 3 choices:
1) fish off the rocks on Nai Yang bay at the southern end of the bay (left side end of the bay when looking at the bay). You will catch small rock fish but nothing large.
2) Contact the local fisherman and negotiate a 1/2 day on their boat. Price should be around 1,000 Baht to 1,500 Baht for 1/2 day. Bring everything you will need, that is, water, food and sun protection. They can provide hand lines and bait for you. There are 2 places on Nai Yang Beach, a) in the middle of the pine park you will see boats parked on the beach, early morning you should be able to catch a few fisherman, b) walk past the restaurants and at the end of the road you will see more fishing boats. Be very careful with the weather which can quickly turn for the worst.
3) You can book a fishing trip with a number of professional companies on Phuket. These companies will provide everything you need for the journey including refreshments, foods, gear, bait and they take you where the fishes are.

Although the answer provided by Phuket Holiday is very good there are lots of other ways to enjoy fishing in Phuket. Fishing of the rocks is great fun but for a bit more action you can get a Longtail out to a reef and do some bottom fishing. This is a cheap fun way to get some quality fishing. You can usually find a longtail who will provide you gear and local knowledge for about 800baht for the morning or afternoon.
Chalong Pier is a good place to meet local fishermen who are usually more than happy to take you out for the price of gas if they are not busy. I met a guy there who takes us trolling to Racha Noi for 8000baht for the day. Private boat & crew & gear and we usually catch about 20 blue tuna and several other good eating fish bottom fishing.
