    You cannot top up Medicare.

Medicare covers ambulance in Queensland for residents, and in Victoria for the elderly, every other state you have to pay. Medicare covers emergency medical in public hospitals, and it does not cover repatriation. Sometimes a hospital will pay for a repatriation back to the patients home country if it is considered a cheaper option than remaining. ie If he were in a Sydney hospital at a cost of $5,000 per day and it was estimated he would be in hospital for 3months, then it would be cheaper for the Australian government to repatriate him to his home country. (if outside of Australia).

I assume if he has a Medicare card he is Australian? He may then require health insurance for the extra's like ambulance, dental, etc.

If he is not Australian and coming into Australia, then he would need travel insurance. about 6 years ago
